If you’ve recently married, divorced, welcomed children or have not checked your beneficiaries in a while, you may wish to update your records. Members who participate in the optional Life & Accident Benefit plan should do the same. As a Relief Association member, you automatically qualify for a Death Benefit that provides money to whomever you name as the beneficiary. The funds are issued after you pass away, so it’s crucial to keep your beneficiaries up to date.
Complete the Designation of Beneficiary form Download, Print and Mail to make changes or call (323) 259-5200.
