Report a Death

The death of a family member can be the most trying time in a person’s life causing great emotional and, perhaps, financial distress. A great deal of uncertainty and fear accompanies a loved one’s death, and coping with the exhausting emotional grief of losing someone you love can be overwhelming.

When reporting a death to Andy, please have the following information on-hand:

  • Date of Birth
  • Time of Death
  • Place of Death
  • Rank of Deceased
  • Pension Classification Date
  • Appointment Date
  • Location of Deceased
  • Funeral Services: Date, Time & Location
